Jim Lydiard is the Chief Strategy Officer of Pine Park Health. Pine Park Health has established itself as a leading value-based primary care organization, solely dedicated to providing service to residents living in Seniors Housing Communities. With >4000 active patients, 3 operating states (CA, AZ, NV), >160 community partners, Pine Park has the dedication, experience, and outcomes necessary to support true aging in place. Through a blend of technology, data innovation, team based care, and on-site delivery, Pine Park Health has proven to increase communities access to care, improve resident satisfaction, all while optimizing the workflows of wellness departments within our community partners.
Prior to joining Pine Park Health, Jim held roles as the CSO with HarmonyCares; the nations largest house call practice, as well as Vice President roles at Strive Health and CareMore/Elevance. CareMore Health is an integrated health care delivery company and subsidiary of Elevance, serving over 200,000 members across 26 states. At CareMore, Jim led the national business division responsible for bringing CareMore’s care delivery model into Seniors Housing communities (SNF, ALF, ILF, CCRC, etc) and those with limited access to outpatient services. Since 2007, Jim has been committed to improving care delivery operations, access, and affordable care models to frail and underserved patients and communities. His organizations, roles, and care teams championed mobile programs in more than 30 states. In his current and past accountable areas, Jim’s responsibilities have included P&L oversight, network strategy and development, product design, analytics & reporting, and growth.
Jim is a routine contributor to various industry forums, such as National Investment Center (NIC), Lincoln Healthcare Leadership, Aging Media Network, and Milken Institute on Aging.
