Jim Lydiard is the CEO at Centered Care and Partner at Senior Living Transformation Company (SLTC).

Centered Care helps senior living communities and healthcare providers work together more effectively to keep seniors out of the hospital. Centered Care is a tech-enabled, care services organization providing best-in-class tools and operational support to implement a more integrated, proactive, and aligned care model. On the front end, Centered care ingests data from the 5-10 disparate point solution data sources used to run a senior living community. Once the data is organized into needs-specific cohorts, community and care programming is activated to enhance the resident individual care plans, and finally care outcomes are realized.

Prior to Centered Care and SLTC, Jim was the Chief Strategy Officer at both Pine Park Health and HarmonyCares. Pine Park Health and HarmonyCares have established themselves as leading value-based primary care organizations, dedicated to providing service to residents living in Seniors Housing Communities and homebound senior populations. Their value-based care teams take on delegated risk from payers, as well as continually perform as one of the highest performing ACO’s in the country. Both PPH/HC combine to serve 45,000 active patients, 18 operating states, >300 community partners.

Prior to 2022, Jim held Vice President roles at Strive Health and CareMore/Elevance. CareMore Health is an integrated health care delivery company and subsidiary of Elevance, serving over 200,000 members across 26 states. At CareMore, Jim led the national business division responsible for bringing CareMore’s full-risk, care delivery model into Seniors Housing communities (SNF, ALF, ILF, CCRC, etc) and those with limited access to outpatient services.

Since 2007, Jim has been committed to improving care delivery operations, access, and affordable care models to frail and underserved patients and communities. In his current and past accountable areas, Jim’s responsibilities have included P&L ownership, network strategy and development, product design, analytics & reporting, and growth.

Jim is a routine contributor to various industry forums, such as National Investment Center (NIC), ASHA, and Milken Institute on Aging.
